When you submit an appointment request, Serenity Springs receives the basic contact and scheduling details you provide. These include your first and last name, phone number, email address, the general reason you are reaching out, your preferred contact method, and the best time to reach you.

The website may also process routine technical information, such as an IP address and request data, to operate the form and help protect it from misuse.

The appointment form does not ask for a personal note or clinical history. Please do not send sensitive clinical details by email. Sending a request does not establish a provider-client relationship or confirm an appointment.

The team uses website information only as reasonably needed to:

  • Review and respond to your appointment request or office question.
  • Coordinate scheduling and follow-up.

Serenity Springs Appointment Request Staff Alerts are low-volume, operational messages for authorized staff members. Each staff recipient gives direct verbal permission before their mobile number is added. People who submit the website appointment form are not enrolled in this internal program and do not receive these alerts.

Message frequency varies with appointment-request volume and may be irregular. Message and data rates may apply. Staff recipients may reply STOP to unsubscribe at any time. Your questions and choices

You may contact the office with a question about information you submitted or to request a correction. Authorized staff members may withdraw consent for internal text alerts by replying STOP or asking the business administrator to remove their number.

Emergency and crisis support
